How to Register
Registration forms can be obtained from the reception desk. New patients on repeat medication will be asked to make an appointment with a doctor.
If you wish to register click on the link below to open the forms. When you have completed all of the details on each form please print off and hand in to the surgery. When you visit the surgery for the first time we will confirm that the details are correct.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you need to fill out and hand in to reception along with the registration form or send the completed forms to: weccg.f81034-results@nhs.net.

Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. You will need to register as a temporary patient. This will allow you to be on the local practice list and still remain a patient of your permanent GP.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.
You cannot register as a temporary patient at a practice in the town or area where you are already registered.
